Invoice order terms
Version 2026-09-26-v1. Review these terms before placing a facility invoice order.
Service provider
EasyPBJ is provided by Joshua King, operating as King & Co. Consulting, 12 Prospect St, Ludlow, MA 01056. Billing and service contact: support@easypbj.app.
Submission-ready file guarantee
For a qualifying product-caused file problem, we will fix it within five business days after the problem is reported and complete usable records and required answers are available, or refund the affected quarter’s fee. Submission-ready means a package for the correct facility and reporting quarter that passes the supported XML checks, with required review completed and source totals reconciled. Complete usable payroll and agency records and accurate review answers are required. The guarantee covers product-caused file problems, not CMS acceptance, filing deadlines, or inaccurate or missing source records. Refunds cover the amount actually paid for the affected facility’s applicable three-month billing period, once per payment; billing periods are separate from CMS reporting quarters. Report a problem through Get help or support@easypbj.app. Business days are Monday–Friday in Eastern Time.
Your purchase
The legal entity named in this order purchases one EasyPBJ facility subscription from King & Co. Consulting for $149 USD every three months. The facility ID identifies the covered facility. Each additional facility needs its own subscription. The billing period starts when the subscription is created, not when Accounts Payable processes payment, and is separate from CMS reporting quarters.
Invoices and payment
Stripe sends invoices to the Accounts Payable email in this order. Each invoice is due within 30 days. Payment must be confirmed before paid features, including new exports, become available for that billing period. An invoice, purchase order, payment in progress, or Accounts Payable approval does not count as payment. Saved records remain available subject to account access and retention rules. No late fee or personal guarantee is part of this order.
Renewal and cancellation
The subscription renews automatically at $149 USD per facility every three months until canceled. Each renewal creates another invoice. The workspace owner can cancel future renewals in Settings through Manage billing and cancellation, or request cancellation at support@easypbj.app before the renewal date. Cancellation does not erase amounts already due, subject to applicable law and any agreed refund. Paid access continues to the end of the paid billing period. Contact support promptly about an invoice error or refund request.
Accounts Payable and purchase orders
The signer confirms that all approvals needed to place this order have been obtained. A PO number is a reference for Accounts Payable; it does not change this order's price, payment deadline, or renewal terms unless King & Co. Consulting agrees in writing. The customer must keep its AP contact current. AP details apply to invoices for this workspace; each facility's PO reference appears on its own subscription invoices.
Service and facility responsibilities
EasyPBJ helps prepare and review PBJ reports. Your facility must verify source files, classifications, hours, totals, and submission results. Automated checks and AI can be wrong. CMS XML export is not CMS acceptance. EasyPBJ prepares the reviewed PBJ XML package; the facility’s authorized submitter uploads it in iQIES and checks the CMS result. EasyPBJ does not guarantee CMS acceptance or universal compliance. The facility remains responsible for timely filing, correcting errors, and keeping appropriate backups.
Electronic agreement and authority
By entering your name and title and selecting both acceptance boxes, you intend to sign this order electronically on behalf of the named customer. You confirm that you have authority to bind that customer and consent to electronic records and invoice delivery. EasyPBJ retains your account ID, verified email, name, title, order details, acceptance time, and the exact agreement accepted. A copy is available in Settings. The named customer, not the individual signer personally, is responsible for payment under this order.
